Even if our vision is to catch all the bugs before the client, and we do good work on the subject there will always be bugs that slip out of our hands.
If we did our best and it happened, there is no need to flog ourselves. On the other hand, they should not be ignored.
The process we have already started with and before each new project we will refresh the conclusions of this process.
